페이션트 제로광견병이 인간에게도 발현되며 그 병이 옮은 사람에게 물린 사람은 90초만에 전염된다. 백신을 만들어 세계를 구하려는 집단이 지하에서 연구를 하며 최초의 감염자, 페이션트 제로를 찾으려고 한다. 모건은 바이러스에 감염된 자에게 물렸지만 변화하지 않은 항체를 가진 사람이다. 그는 물리고 나서 감염자들과 대화를 할 수 있게 되었고 생포한 감염자들과의 대화를 통해 페이션트 제로를 찾으려 한다. 하지만 지능을 가진 감염자들 역시 남은 사람들에 대한 반격을 꾀한다.
