사탄의 테러존(John Reilly: 제프리 콤브스 분)은 부인 수잔(Susan Reilly: 바바라 크램프톤 분), 딸 레베카(Rebecca Reilly: 제시카 돌라하이드 분)와 함께 아버지로부터 물려받은 이태리의 한 고성을 방문한다. 하지만 그의 가족은 행복한 웃음을 잃어버린지 오래인데 그것은 그가 가족들을 데리고 차를 몰고가다 트럭과 부딪혀 아들 제이제이(JJ: 알렉산드로 세바스찬 사타 분)가 사망하고 딸 레베카가 실명당한 악몽 같은 과거의 기억 때문이다. 더구나 사고 후 성기피 증세를 보이는 아내 수잔은 고성에 와서도 남편과 방을 같이 쓰는 것을 피한다. 한편, 성의 지하 감옥에는 쇠사슬에 묶여 끔찍한 고문을 당하며 괴물같은 형상으로 변해버린 정체불명의 사내가 신음하고 있는데. 눈이 먼 대신 귀가 민감한 레베카는 성안에 다른 누군가가 있는 것을 감지하고 부모에게 알린다. 처음에 이 사실을 믿지 않던 존도 시간이 지날 수록 성안에 심상치 않은 일이 일어나는 것을 눈치채고 경찰에 신고하지만 아무런 도움을 얻지 못한다. 어느날 밤 존은 부인 수잔에게 사랑을 갈구하지만 수잔은 냉정하게 거절한다. 실의에 빠진 존은 시내의 술집에서 젊은 이태리 여인을 만나게 되고 함께 성의 지하감옥에 들어가 뜨거운 정사에 몰입한다. 그러나 아내를 너무나 사랑하는 존은 도중에 포기하고 그녀를 돌려 보낸다. 그러나 지하 감옥에 숨어서 정사를 지켜보던 반인반수의 괴물이 집으로 돌아가는 이태리 여인을 덮치는데.
