スクリーム連続猟奇殺人事件から25年が経ったウッズボローの街。ティーンエイジャーの少女タラのもとに、謎のゲームに誘う不気味な電話が掛かってくる。恐怖心を抱き玄関から外へ出ようとしたタラは、待ち構えていたゴーストフェイスの仮面姿の人物に襲われてしまう。この事件から、かつてと同じく殺人鬼が街の若者たちを狙っていることを知ったデューイは、シドニーやゲイルとともに再びゴーストフェイスに立ち向かう。
